Dentin Hypersensitivity Diagnosis and Management
Summary
Dentin hypersensitivity is characterised by a transient, sharp pain arising from exposed dentin in response to thermal, tactile, osmotic or chemical stimuli. It affects a substantial proportion of adults worldwide and can have a marked impact on eating, drinking and oral hygiene practices. The prevailing explanation for symptom generation is the hydrodynamic theory, which attributes pain to fluid shifts within open dentinal tubules that activate pulpal mechanoreceptors. Accurate diagnosis requires careful exclusion of caries, cracked tooth syndrome, pulpitis and restorative defects, typically by applying standardised stimuli and recording patient responses on a pain scale. Management begins with identifying and controlling predisposing factors such as enamel erosion, gingival recession, abrasive brushing and acidic diets. First-line treatment comprises non-invasive home-care measures, including desensitising toothpastes and fluoride rinses. Persistent cases may benefit from in-office applications of occluding agents (oxalates, glutaraldehyde/HEMA, resin sealants) or low-level laser therapy. In refractory situations, adhesive restorations and mucogingival surgery can establish a physical barrier against stimuli. Lifelong maintenance and patient education remain central to long-term success, with emerging therapies poised to refine this multi-modal approach.

A novel desensitising strategy has been developed using a self-assembled tannic acid–iron(III) complex film deposited by rapid dip-coating. This coating effectively occluded human dentinal tubules, resisted brushing abrasion and promoted in situ hydroxyapatite remineralisation, pointing to a facile in-office application. Another study employed an extended in vitro protocol—incorporating multiple applications, thermal ageing and saliva exposure—to compare a hydroxyapatite-containing toothpaste with formulations based on potassium nitrate and arginine–calcium carbonate. Results demonstrated that the hydroxyapatite paste offered the greatest reduction in dentin permeability in the absence of saliva, whereas the arginine–calcium carbonate paste was most effective with salivary proteins present. These findings underscore the importance of simulating clinical conditions in laboratory testing and suggest tailored formulation strategies for optimal tubule occlusion.
