Summary

Depressive symptoms are highly prevalent among individuals with knee osteoarthritis, compounding pain, functional limitation and quality-of-life decline. The convergence of chronic joint pain, mobility impairment and psychosocial stressors often precipitates or exacerbates mood disturbances. Depressive symptoms in this context encompass persistent low mood, anhedonia and cognitive changes that may interfere with pain coping and rehabilitation adherence. Biopsychosocial models emphasise that inflammatory mediators, altered pain processing and social isolation interact to sustain both joint pathology and affective distress. Recognising and addressing depression in knee osteoarthritis is therefore vital for optimising pain management, functional recovery and overall wellbeing on a global scale.

Recent studies have quantified the rising burden of psychological comorbidities in osteoarthritis at a national level. A large cross-sectional analysis of health-insurance data in Germany documented that approximately one in eleven patients with primary knee osteoarthritis receives a concurrent diagnosis of a mood or affective disorder. Between 2009 and 2019, the proportion of individuals with depressive or other psychiatric diagnoses increased by nearly 18 per cent, underscoring a growing need for routine screening. The study highlights the importance of integrating mental-health assessment into standard osteoarthritis care pathways and supports the development of interdisciplinary treatment models that combine orthopaedic, physical and psychological interventions.
