Summary
South Asian populations bear a disproportionately high burden of type 2 diabetes, with prevalence rates often two to three times higher than those in White European groups. This disparity manifests at lower levels of adiposity and earlier ages, driven by a combination of central fat distribution, genetic predisposition, and rapid transitions in dietary and physical-activity patterns. Urbanisation and shifts towards energy-dense diets rich in refined carbohydrates and saturated fats exacerbate insulin resistance and beta-cell stress, while traditional protective factors such as communal physical labour decline. Epidemiological studies highlight that many South Asian adults develop diabetes at body mass indices (BMIs) conventionally considered healthy in other ethnicities, underscoring the need for ethnicity-specific risk assessment and screening thresholds. Lifestyle interventions incorporating dietary modification, increased physical activity and behavioural support have shown promise, particularly when culturally adapted and delivered through family or community networks. Yet achieving sustained weight loss and metabolic improvement remains challenging owing to socioeconomic barriers, linguistic diversity and differing health beliefs. A combined approach that integrates clinical surveillance with community engagement and digital tools may offer the greatest potential to stem rising diabetes rates among South Asian communities worldwide.
