Summary
Diabetes mellitus is closely linked to a spectrum of oral health challenges that arise through complex metabolic and immunological pathways. Chronic hyperglycaemia fosters formation of advanced glycation end-products and heightens systemic inflammation, which in turn compromise periodontal tissues. Impaired neutrophil function, altered collagen metabolism and microvascular changes render the periodontium more susceptible to destruction. Conversely, severe periodontal inflammation can exacerbate insulin resistance by serving as a chronic inflammatory reservoir, creating a bidirectional relationship. Common manifestations include gingival bleeding, periodontitis, increased dental caries and accelerated tooth loss. Xerostomia and candidal overgrowth may also affect salivary function and mucosal integrity. Collectively, these conditions diminish quality of life, complicate glycaemic control and impose substantial economic and social burdens. Optimal management demands integration of medical and dental care, emphasising regular professional assessment, meticulous plaque control, patient education and tailored periodontal interventions. Emerging evidence underlines the importance of interdisciplinary approaches to reduce the global impact of diabetes-related oral disease and to improve overall metabolic and oral health outcomes.

Recent studies have demonstrated that adults with poorly controlled diabetes face significantly higher odds of moderate to severe periodontal disease compared with those without diabetes. Analysis of large-scale health survey data revealed a stepwise increase in periodontal prevalence as glycated haemoglobin levels rise, with the greatest risk observed in individuals with HbA1c above nine per cent. This research also identified protective factors, including routine annual dental visits and abstention from tobacco, which mitigate periodontal risk even in the context of diabetes. Findings underscore the critical role of glycaemic control and preventive dental care in attenuating inflammatory periodontal outcomes among diabetic populations.
