Effective management strategies centre on comprehensive risk assessment, pressure redistribution, advanced wound care and targeted revascularisation. Early identification of sensory loss and ischaemia facilitates timely interventions. Pressure redistribution, or offloading, remains a cornerstone of both prevention and healing, employing a range of devices from total contact casts to custom-made insoles. Adjunctive wound therapies include debridement, moisture-balance dressings, negative pressure wound therapy and biological agents. Restoration of perfusion via endovascular or surgical revascularisation complements conservative measures by addressing tissue oxygenation. Patient education, adherence monitoring and multidisciplinary coordination underpin long-term outcomes, aiming to reduce recurrence, limb loss and healthcare burden on a global scale.
