Summary
Diabetic kidney disease (DKD) represents a leading cause of chronic kidney failure worldwide, arising from hyperglycaemia-induced damage to glomerular and tubular structures. Early stages are often asymptomatic, with microalbuminuria heralding initial glomerular injury. As disease advances, thickening of the glomerular basement membrane, mesangial expansion and podocyte loss contribute to progressive proteinuria and decline in glomerular filtration. While clinical markers such as albuminuria and estimated glomerular filtration rate (eGFR) guide diagnosis and staging, they lack specificity for distinguishing pure diabetic nephropathy from concurrent non-diabetic renal diseases. Renal biopsy remains the definitive method for histopathological characterisation, revealing patterns of nodular glomerulosclerosis, interstitial fibrosis or alternative glomerulopathies. Precise biopsy-driven diagnosis is crucial to tailor therapy—ranging from renin–angiotensin blockade and sodium–glucose cotransporter-2 inhibition to immunosuppression for coexisting immune-mediated lesions—and to refine prognostic models. Increasingly, non-invasive biomarkers and machine-learning algorithms are devised to predict underlying histology and optimise patient selection for biopsy. Together, these advances underscore the global imperative to integrate histological assessment with emerging diagnostic tools in diabetic renal care.

Recent studies have evaluated novel serological and clinical parameters to improve non-invasive diagnosis and prognostication. A multicentre analysis of patients with type 2 diabetes and persistent proteinuria has demonstrated that circulating anti-phospholipase A₂ receptor antibodies reliably identify membranous nephropathy superimposed on diabetic kidney disease. This immunological marker achieved high diagnostic accuracy, offering an alternative when biopsy is contraindicated. Separately, investigators have validated a clinical model incorporating diabetic retinopathy status and duration of diabetes to predict the likelihood of true diabetic nephropathy in non-biopsied individuals. The presence of retinopathy and model-based risk estimates were independently associated with accelerated progression to end-stage renal disease, supporting their use in risk stratification and trial enrichment strategies.

Technical terms
Diabetic nephropathy (DN): Kidney damage resulting specifically from long-standing diabetes, characterised by glomerulosclerosis and albuminuria.
Non-diabetic renal disease (NDRD): Any renal pathology in a diabetic patient unrelated to diabetic microvascular injury, such as IgA nephropathy or membranous nephropathy.
Proteinuria: Excessive excretion of protein in the urine, often quantified by albumin-to-creatinine ratio or 24-hour collection.
Estimated glomerular filtration rate (eGFR): A calculated index of kidney function based on serum creatinine, age, sex and race.
Anti-phospholipase A₂ receptor (anti-PLA₂R) antibody: An autoantibody targeting podocyte antigens, used as a biomarker for idiopathic membranous nephropathy.
Machine-learning model: A computational algorithm trained on clinical and laboratory data to predict underlying histological diagnoses.
