Diagnosis and Management of Histoplasmosis Infections
Summary
Histoplasmosis is a fungal infection caused by inhalation of Histoplasma capsulatum spores, with a spectrum ranging from asymptomatic pulmonary colonisation to life-threatening disseminated disease. Clinical suspicion should be guided by epidemiological history, including residence in or travel to endemic regions, and exposure to bat or bird droppings. Diagnosis relies on a combination of imaging, fungal culture, histopathology, serology and antigen detection assays in urine or serum, which provide rapid sensitivity in disseminated cases. Molecular techniques, such as PCR and next-generation sequencing, are increasingly employed to enhance specificity and detect resistant strains. Management depends on disease severity: severe or disseminated infections warrant induction with amphotericin B formulations, followed by long-term itraconazole therapy, whereas mild to moderate pulmonary disease can often be treated with itraconazole alone. In immunocompromised patients, maintenance therapy and regular monitoring of drug levels are crucial to prevent relapse. Emerging approaches include point-of-care antigen assays and novel triazoles, offering improved access and tolerability. Global challenges remain in distinguishing histoplasmosis from tuberculosis in endemic areas, ensuring timely treatment and expanding diagnostic capacity in resource-limited settings.
