Summary

Tuberculous lymphadenitis, the most common form of extrapulmonary tuberculosis, presents predominantly as chronic, non-tender lymph node enlargement, often in cervical chains. Initial evaluation combines clinical assessment with imaging, most commonly ultrasound or computed tomography, to characterise nodal architecture, vascularity and necrosis. Definitive diagnosis relies on cytological or histological sampling via fine-needle aspiration cytology (FNAC) or core/excisional biopsy, supported by acid-fast staining and culture. Rapid molecular assays have revolutionised detection of Mycobacterium tuberculosis and rifampicin resistance, shortening time to treatment. Management centres on a standard six-month regimen of first-line antitubercular drugs, adjusted for drug susceptibility and patient comorbidities. Surgical intervention is reserved for diagnostic uncertainty, significant abscess or sinus formation, drug resistance and paradoxical reactions under therapy. Follow-up imaging and clinical review are essential to distinguish residual nodes from treatment failure or relapse. Immune profiles at the site of infection reveal diminished cytotoxic T-cell and natural killer cell activity, suggesting local immunomodulation that may inform adjunctive therapies. Globally, tuberculous lymphadenitis poses diagnostic challenges in resource-limited settings, where combined use of cytology, molecular testing and context-appropriate imaging ensures timely management. Advances in point-of-care diagnostics and precision medicine promise to refine treatment duration, reduce morbidity and curb transmission of resistant strains, underlining the importance of integrated clinical and laboratory pathways.
