Summary
Assessing diet quality in older adults has become a cornerstone of public health nutrition, given the demographic shift towards ageing societies worldwide. Age-related physiological changes, comorbid conditions and psychosocial factors combine to influence dietary intake, nutrient bioavailability and ultimately health outcomes such as sarcopenia, frailty and cognitive decline. Traditional approaches to evaluation have relied on nutrient-based analyses and adherence to dietary guidelines, but recent work has pivoted towards dietary pattern analysis and composite indices that capture overall dietary quality. These instruments balance ease of administration with sensitivity to cultural and socioeconomic contexts. Common methods include Food Frequency Questionnaires (FFQs) tailored to older populations, 24-hour recalls and brief screening tools. Composite indices—such as the Diet Quality Index for the Elderly (DQI-E), the Revised Diet Quality Index (DQI-R) and the Brazilian Healthy Eating Index—translate complex dietary data into scores reflecting adherence to recommended consumption of fruits, vegetables, whole grains, lean proteins and limiting saturated fats, sugars and sodium. Emerging methodologies integrate cluster analysis to identify subgroups with distinct dietary patterns and employ multivariate regression to link these patterns with sociodemographic, health and lifestyle factors. The ultimate aim is to inform targeted interventions, guide clinical practice and shape public health policy by identifying vulnerable subpopulations and modifiable determinants of poor diet quality.
