Summary
Childhood represents a critical window for shaping lifelong cardiometabolic trajectories. Emerging evidence links dietary patterns established in early years to the development of obesity, dyslipidaemia, insulin resistance and hypertension later in life. Interventions targeting macronutrient quality—particularly the replacement of saturated fats with polyunsaturated fats—alongside increased fruit, vegetable and whole-grain intake have been shown to modulate lipoprotein profiles, reduce low-density lipoprotein cholesterol and attenuate age-related rises in insulin resistance. Family-based, individualised counselling delivered over several years can shift food choices towards lower added sugars and higher dietary fibre, with sustained effects on circulating fatty acid composition and lipoprotein subclasses. Integrating multi-omic approaches and composite exposure metrics now permits exploration of how diet interacts with other lifestyle and environmental factors to influence serum metabolites from childhood into adolescence. Globally, culturally tailored dietary strategies offer practical routes to curb the burgeoning prevalence of paediatric obesity and its sequelae, underscoring the importance of early nutritional education, policy-driven food reformulation and community engagement in fostering heart-healthy habits.

Recent longitudinal analyses have utilised an exposome score to capture combined effects of diet, physical activity, sleep, air pollution and socioeconomic status on the evolving serum metabolome from childhood into adolescence. In a cohort followed over eight years, a higher composite exposome score predicted alterations in lipid-related and amino acid metabolites beyond those explained by individual exposures alone. Notably, several metabolites uniquely associated with the combined score emerged as potential early biomarkers of cardiometabolic risk, illustrating the value of integrated exposure assessment in understanding how dietary and non-dietary factors jointly shape metabolic health during growth.
