Diffuse idiopathic skeletal hyperostosis (DISH) is a non-inflammatory enthesopathy characterised by progressive ossification of spinal ligaments and the formation of flowing anterior osteophytes. It predominantly affects individuals over 50 years of age, with a higher prevalence in men, and has been linked to metabolic disorders such as obesity, type 2 diabetes and hypertension. Radiographically, diagnosis requires the presence of contiguous ossifications spanning at least four vertebral levels without significant disc space narrowing or sacroiliac changes typical of inflammatory spondyloarthropathies. Clinically, many individuals remain asymptomatic, but some experience axial stiffness, reduced spinal mobility, and pain. Complications include an elevated risk of unstable spinal fractures even after low-energy trauma, spinal canal narrowing with cord impingement, dysphagia and, in rare cases, airway obstruction. Management is multidisciplinary: conservative measures focus on analgesia, physical rehabilitation and optimisation of metabolic risk factors; surgical intervention is reserved for neurological compromise, decompression of haematoma or resection of osteophytes causing dysphagia; and fracture management often involves stabilisation and decompression to prevent delayed neurological deterioration.
