Summary

Accurate assessment of post‐stroke disability is central to guiding rehabilitation, evaluating therapeutic interventions and informing health policy. Disability after stroke encompasses impairments in motor, cognitive and communicative functions, as well as limitations in activities of daily living (ADL) and participation in social roles. Measurement tools must balance sensitivity to change with ease of administration, cultural applicability and psychometric robustness. Widely used scales such as the modified Rankin Scale and the Barthel Index offer complementary perspectives on global functional status and independence in ADL. Clinimetric evaluation of these instruments—covering validity, reliability, responsiveness and feasibility—underpins their adoption in clinical trials and routine practice. Recent advances include digital data collection methods, utility‐based weighting of ordinal scales to reflect patient‐centred outcomes and refined cut-off criteria to harmonise scale grades. These developments aim to capture the full spectrum of recovery, support stratified care pathways worldwide and enable meaningful comparisons across studies and health systems.
