Summary
Disseminated gonococcal infection (DGI) arises when Neisseria gonorrhoeae breaches mucosal barriers and enters the bloodstream, producing a spectrum of systemic manifestations. Although primary infection typically involves the urogenital, pharyngeal or rectal mucosa, a small fraction of cases progress to disseminated disease. Clinical features often include fever, dermatitis, tenosynovitis and migratory polyarthralgia; less common but more severe complications encompass septic arthritis, endocarditis, meningitis and osteomyelitis. Host factors such as complement deficiencies and polymorphisms in immune-regulatory genes can predispose to invasive disease. Antimicrobial resistance has become a pressing concern, with reduced susceptibility to extended-spectrum cephalosporins and macrolides challenging current treatment guidelines. Rapid nucleic acid amplification tests enable early detection, while synovial fluid or blood cultures remain essential for pathogen isolation and susceptibility assessment. Genomic surveillance and phylogenetic analyses are shedding light on the spread of resistant lineages, informing both clinical management and public health interventions. Global control of DGI requires coordinated efforts in sexual health education, routine screening of at-risk populations, and stewardship of existing antibiotics alongside development of novel therapeutics and vaccines.
