Drug-Resistant Epilepsy Management and Prognosis
Summary
Drug-resistant epilepsy (DRE) affects approximately one third of people with epilepsy and is defined by the failure to achieve sustained seizure freedom despite trials of two or more well-chosen antiseizure medications. Patients with DRE face higher risks of injury, comorbidity and premature mortality, underlining the need for prompt recognition and tailored interventions. Management strategies combine optimised pharmacotherapy, neurosurgical evaluation and emerging neuromodulation or dietary therapies. Prognosis hinges on identifying clinical and radiological predictors of treatment response, such as age at onset, seizure type, imaging abnormalities and early treatment failure. Robust prognostic models now integrate longitudinal data—seizure frequency trends and treatment history—to refine individualised risk stratification. Early referral for epilepsy surgery, when a focal structural lesion or clear epileptogenic zone is present, can offer seizure remission rates exceeding 60 %, whereas neuromodulation may benefit those unsuitable for resection. Across low-resource and high-income settings, standardised definitions and treatment algorithms are essential to reduce disparities in care and improve long-term outcomes.
