Ectopic Pituitary Adenoma Diagnosis and Management
Summary
Ectopic pituitary adenomas are rare neuroendocrine tumours arising outside the sella turcica along the embryonic migratory path of pituitary cells. They can present anywhere from the nasopharynx to the clivus or sphenoid sinus, often with hormone hypersecretion or mass-effect symptoms. Diagnosis is challenging owing to normal intrasellar anatomy on routine imaging and non-specific endocrine tests. Multimodal assessment—including high-resolution magnetic resonance imaging, computed tomography, nuclear medicine scans and targeted suppression tests—is essential to localise the lesion and distinguish it from primary resistance syndromes or chordomas. Histopathological confirmation with immunohistochemistry remains the diagnostic gold standard. Management strategies are individualised and may include endoscopic transsphenoidal resection, medical therapy with somatostatin analogues or dopamine agonists for functional tumours, and radiotherapy for invasive or residual disease. Recent advances in epigenomic profiling and hybrid PET/MRI techniques have improved diagnostic confidence and guided precision treatment planning.
