Summary
Electroencephalography (EEG) is a cornerstone of epilepsy management, providing real-time measurement of cortical electrical activity for diagnosis, monitoring and treatment guidance. In routine practice, conventional EEG employs a full montage of 19–21 electrodes placed according to standardised systems, enabling detection of interictal epileptiform discharges and ictal patterns. Recent technological innovations have broadened its applications beyond the neurology ward and specialised laboratories. Portable and point-of-care EEG devices now permit rapid assessment in emergency departments, community hospitals and prehospital settings, reducing time to diagnosis and patient transfers. Reduced-lead montages and wireless headsets have been shown to preserve diagnostic accuracy while lowering set-up time and resource requirements. Automated algorithms and machine-learning methods are also being integrated to provide real-time seizure burden analysis and bedside alerts for status epilepticus, streamlining clinical workflows and facilitating timely intervention. Continuous EEG monitoring in intensive care units and remote home-monitoring approaches further extend the reach of EEG, offering longitudinal seizure surveillance and therapeutic evaluation in ambulatory patients. Collectively, these advances underscore the global significance of EEG as an adaptable tool for improving outcomes in people with epilepsy.

Electroencephalography (EEG): A non-invasive technique for recording electrical potentials generated by neuronal populations using scalp electrodes.
Nonconvulsive status epilepticus (NCSE): A prolonged seizure state characterised by altered consciousness without overt motor convulsions, diagnosed primarily through EEG patterns.
Point-of-care EEG (pocEEG): A portable, rapid-setup EEG system designed for bedside or field use, often with reduced electrode montages.
Electrode montage: The spatial arrangement of electrodes on the scalp; full montages use 19–21 channels, while reduced montages use fewer electrodes for faster application.
Artefact: Extraneous signals in EEG recordings arising from muscle activity, movement or equipment interference, which can obscure cerebral activity.
