Endometrial Ablation Techniques for Menorrhagia Management

Menorrhagia, defined as excessive menstrual blood loss that interferes with a woman’s physical, emotional and social well‐being, remains one of the most common gynaecological complaints worldwide. Endometrial ablation encompasses a suite of minimally invasive methods designed to destroy or remove the endometrial lining in order to reduce menstrual bleeding. Techniques range from hysteroscopic resection, which mechanically removes tissue under direct vision, to non‐resectoscopic second‐generation approaches such as bipolar radiofrequency ablation, thermal balloon therapy and hydrothermal ablation, each offering shorter operative times and the possibility of outpatient treatment. These procedures aim to achieve amenorrhoea or a significant reduction in bleeding, with overall success rates typically exceeding 70 percent at two years. Patient selection is guided by endometrial thickness, presence of intracavitary pathology and reproductive desires. Long‐term follow-up series indicate that reintervention, including repeat ablation or hysterectomy, occurs predominantly within the first two years, with cumulative rates ranging from 10 to 30 percent. Emerging strategies explore combining ablation with adjunctive hormonal devices to forestall endometrial regeneration. In low-resource settings, device portability and ease of use render some second-generation modalities particularly advantageous. Economic evaluations demonstrate favourable cost profiles compared with hysterectomy, especially when outpatient care pathways are utilised. Overall, endometrial ablation offers a uterus-sparing, cost-effective option that can be tailored to individual anatomy and clinical circumstances, with ongoing refinements aimed at improving durability and widening access.
