Endoscopic Techniques for Superficial Esophageal Neoplasia

Summary

Superficial esophageal neoplasia encompasses early mucosal lesions with malignant potential that remain confined to the epithelium or submucosa. Timely detection and precise characterisation of these lesions are critical to enable minimally invasive treatment and improve patient outcomes. Conventional white-light endoscopy provides broad visualisation but may miss subtle lesions or misjudge invasion depth. Image-enhanced endoscopy modalities such as narrow-band imaging, linked colour imaging and blue light imaging exploit specific light wavelengths to enhance mucosal and vascular contrast, improving lesion detection. Magnifying endoscopy allows detailed evaluation of microvascular patterns, aiding depth estimation and therapeutic planning. Endoscopic ultrasonography offers cross-sectional imaging of the oesophageal wall to differentiate mucosal from submucosal invasion, guiding indication for endoscopic resection or surgery. Endoscopic submucosal dissection has emerged as the preferred technique for en bloc removal of superficial neoplasms, combining diagnostic and curative aims. Novel optical biopsy tools, including confocal laser endomicroscopy and three-dimensional transparency-enhancing technology, promise in vivo histopathological assessment and volumetric specimen analysis. Ongoing efforts focus on improving diagnostic accuracy, reducing procedural invasiveness and integrating artificial intelligence for real-time decision support. Collectively, these advances aim to standardise the early management of superficial oesophageal neoplasia worldwide, reducing disease burden and preserving organ function.
