Eosinophilic Dermatological Conditions and Treatment Approaches

Summary

Eosinophilic dermatological conditions comprise a diverse group of skin disorders in which eosinophils—granulocytic leukocytes implicated in type 2 immunity—accumulate within cutaneous tissues and drive inflammation. Common entities include eosinophilic annular erythema, eosinophilic cellulitis (Wells syndrome), eosinophilic pustular folliculitis and eosinophilic fasciitis. Clinically, these disorders manifest as pruritic plaques, annular or pustular eruptions and, in deeper involvement, indurated or scleroderma-like changes. Histopathology often reveals perivascular and interstitial eosinophil‐rich infiltrates sometimes accompanied by characteristic ‘flame figures’. Traditional management has centred on systemic and topical corticosteroids, dapsone or antimalarial agents, but relapses and treatment resistance remain common. Recent advances in understanding the immunobiology of eosinophils and type 2 cytokine networks have paved the way for targeted therapies—ranging from tetracycline antibiotics with anti‐inflammatory effects to monoclonal antibodies against interleukin-4/13 pathways—offering improved disease control and steroid-sparing potential.
