Summary
The HIV/AIDS epidemic remains a critical global health challenge marked by considerable regional and population-specific variation. Overall prevalence has stabilised or declined in many settings owing to widespread antiretroviral therapy (ART) roll-out and prevention initiatives, yet incidence persists in key populations and geographic hotspots. Advances in spatial analysis have uncovered fine-scale heterogeneity, revealing localised “hotspots” of transmission that national averages obscure. Prevention strategies now combine behavioural interventions (safe-sex education, condom distribution), structural measures (social protection, poverty alleviation), and biomedical approaches (treatment as prevention, pre-exposure prophylaxis, harm-reduction for people who inject drugs). Universal test-and-treat policies aim to diagnose and suppress viral replication rapidly, reducing onward transmission. Integration of social determinants into epidemic monitoring, alongside geotargeted resource allocation, is essential to sustain progress towards global targets and address persistent inequities in HIV burden.

Generalised geo-additive modelling of national survey data in Ethiopia has mapped HIV prevalence across administrative zones, pinpointing rural and urban disparities and identifying high-burden “hotspots” linked to sexually transmitted infections, contraceptive use and education level. This granular mapping supports strategic allocation of prevention and treatment resources at subnational levels. A continent-wide high-resolution analysis estimated adult HIV prevalence at a 5×5 km scale across sub-Saharan Africa from 2000 to 2017, uncovering substantial intra-country variation in both prevalence and temporal trends. By exposing local areas where prevalence remained high despite national declines, this work provides a precision tool for directing combination prevention efforts where they are most needed.
