Secular trends vary by region: some East Asian populations show increasing prevalence tied to rapid urbanisation and evolving risk factor profiles, while evidence from large multicentre studies in Europe and North America suggests cohort effects with later-born generations at somewhat lower age-specific risk. Projections indicate that even with declining incidence, total case numbers may grow substantially in coming decades, posing challenges for health systems and social care. This evolving landscape underscores the need for ongoing surveillance of incidence and prevalence, alongside targeted interventions to sustain and accelerate declines in age-specific risk, particularly in low- and middle-income settings.

This study demonstrates that stable methodology across geographical sites can reveal genuine cohort effects rather than artefacts of changing diagnostic criteria. In a separate investigation of 85-year-olds in Sweden, identical assessments conducted 22 years apart found a fall in dementia prevalence from 29.8% to 21.7%. Increased educational attainment and improved post-stroke care in later-born cohorts accounted for most of the decrease, highlighting the impact of cognitive reserve and enhanced vascular management on dementia risk.
