Epidemiology and Antimicrobial Resistance of Staphylococcus aureus

Summary

Staphylococcus aureus is a versatile pathogen responsible for a spectrum of conditions ranging from superficial skin infections to invasive diseases such as bacteraemia and endocarditis. Central to its public health impact is the emergence and global dissemination of methicillin-resistant S. aureus (MRSA), driven by acquisition of the mecA gene carried on the SCCmec element. MRSA has given rise to distinct hospital-associated and community-associated lineages, each characterised by unique virulence factors, including Panton–Valentine leukocidin (PVL) and enhanced biofilm formation. Variations in clonal complex prevalence reflect regional antibiotic usage patterns and infection-control practices. High-burden settings often report multi-drug resistance spanning beta-lactams, macrolides and tetracyclines, while glycopeptides remain among the few reliably active agents. Surveillance integrating molecular typing, antimicrobial susceptibility testing and epidemiological data is essential to track the evolution of resistance, guide empirical therapy and inform stewardship interventions. Global efforts now focus on minimising transmission through rigorous hygiene measures, rational antibiotic use and development of novel therapeutics and vaccines.

Recent studies have explored the emergence of virulent clonal lineages in paediatric bacteraemia. Longitudinal analysis revealed a marked increase in PVL-encoding gene carriage among S. aureus isolates over two decades, coinciding with the rise of the CC152 lineage. This lineage exhibits robust biofilm production and heightened virulence in invertebrate infection models, underscoring its potential to complicate treatment. Such findings highlight the importance of monitoring lineage-specific traits that may influence clinical outcome and therapeutic efficacy.
