Epidemiology and Clinical Management of Blastomycosis
Summary
Blastomycosis is an invasive fungal infection caused by thermally dimorphic species of the genus Blastomyces, predominantly Blastomyces dermatitidis and the more recently identified Blastomyces gilchristii. Endemic in regions of North America and more sporadically observed in parts of Africa and India, the disease arises following inhalation of infectious conidia from environmental reservoirs such as moist soil and decaying organic matter. Clinical manifestations range from subclinical or self-limited pulmonary involvement to severe pneumonia, acute respiratory distress syndrome or disseminated disease affecting skin, bone, genitourinary tract and, infrequently, the central nervous system. Diagnosis relies on culture, histopathology and rapid antigen detection in serum or urine, although cross-reactivity with other endemic mycoses can complicate interpretation. Effective management depends on disease severity: severe or central nervous system involvement is treated initially with lipid formulations of amphotericin B, followed by step-down to oral azoles, most commonly itraconazole. For mild to moderate pulmonary or extrapulmonary disease, oral itraconazole is the first-line therapy. Delays in diagnosis and treatment contribute to morbidity and mortality, emphasising the need for heightened clinical suspicion in endemic and emerging regions. Ongoing surveillance and improved environmental mapping are critical to delineating risk zones and guiding public health interventions.

Lipid formulation of amphotericin B: A carrier-based preparation of amphotericin B that reduces toxicity while retaining antifungal potency.
Azoles: A class of antifungal drugs (e.g., itraconazole) that inhibit ergosterol synthesis in fungal cell membranes.
