Summary

Chronic low back pain is one of the foremost contributors to years lived with disability worldwide, affecting an estimated 20–23 percent of the adult population at any given time. Prevalence rises steadily from early adulthood, peaking in mid‐life, and displays notable variation across regions owing to differences in occupational exposures, lifestyle factors and health-care access. The condition arises from a complex interplay of structural spinal changes, neuromuscular dysfunction and psychosocial influences. Management guidelines uniformly advocate a biopsychosocial and multidisciplinary approach, combining patient education, tailored exercise programmes, spinal manipulation, analgesic regimens and cognitive behavioural techniques. Stratified care models that assess prognostic indicators enable personalised treatment pathways, aiming to restore function and minimise recurrences. Preventive strategies at a population level focus on ergonomic workplace design, promotion of physical activity and early rehabilitation. Despite modest declines in age-standardised rates over recent decades, the absolute burden continues to grow in line with population ageing and rising rates of obesity, placing sustained demands on health systems and economies across both high-income and resource-limited settings.
