Summary
Hepatocellular carcinoma (HCC) ranks among the leading causes of cancer mortality worldwide, arising predominantly in the context of chronic liver disease. Its incidence varies markedly by region, driven by differences in prevalence of hepatitis B and C viruses, exposure to aflatoxin, alcohol consumption and emerging contributions from non-alcoholic fatty liver disease linked to obesity and diabetes. Age-standardised rates are highest in East Asia and sub-Saharan Africa but are rising rapidly in Europe and North America. Surveillance programmes targeting at-risk populations—typically adults with cirrhosis—employ ultrasonography with or without measurement of α-fetoprotein to detect early lesions amenable to curative therapies. Curative options include surgical resection, percutaneous ablation and liver transplantation, with selection largely guided by tumour number, size and underlying liver function as defined by staging systems such as the Barcelona Clinic Liver Cancer classification. For intermediate-stage disease, transarterial chemoembolisation remains standard, whereas systemic management has shifted from first-generation tyrosine kinase inhibitors to combination immunotherapy, harnessing immune checkpoint blockade with or without anti-angiogenic agents. Long-term control relies on antiviral therapy for hepatitis-related disease, vaccination against hepatitis B and broader lifestyle interventions to address metabolic risk. A multidisciplinary approach uniting hepatologists, interventional radiologists, surgeons and oncologists is now essential to optimise outcomes.

International analyses have mapped global incidence and mortality trends over recent decades, revealing a nine-fold variation between countries and a marked negative correlation with indices of socioeconomic development. Low-income regions continue to bear the highest burden, but several high-income countries have experienced sharp rises in HCC rates, reflecting the growing impact of metabolic liver disease. Detailed join-point regression studies have pinpointed countries with the steepest annual increases, highlighting the need for targeted prevention and early detection in emerging high-risk populations. These findings underscore shifting epidemiological patterns and reinforce the importance of adapting surveillance and public health strategies to changing risk profiles.
