Equine Temporomandibular Joint Diagnostics and Treatment
Summary
The equine temporomandibular joint (TMJ) is a complex, diarthrodial articulation comprising two synovial compartments—the discotemporal and discomandibular joints—separated by a fibrocartilaginous disc. Disorders affecting this joint, including osteoarthritis, septic arthritis, intra-articular disc pathology and traumatic injury, can impair mastication, bit acceptance and overall performance. Initial assessment combines clinical examination with conventional radiography and ultrasonography to identify bony irregularities and gross soft-tissue changes. Advanced imaging modalities such as computed tomography and magnetic resonance imaging afford high-resolution evaluation of osseous architecture and disc integrity, while arthrocentesis and contrast arthrography permit targeted sampling and visualisation of intra-articular structures. Minimally invasive arthroscopic techniques, increasingly performed under standing sedation, enable both confirmation of diagnosis and direct therapeutic intervention, including debridement and intra-articular medication delivery. Medical management centres on anti-inflammatory therapy, joint lavage and physio-therapeutic support. In more severe or refractory cases, surgical approaches—such as condylectomy with meniscectomy or selective arthrodesis—offer durable resolution of clinical signs. A multimodal strategy, integrating evolving diagnostic and therapeutic tools, underpins tailored treatment plans that aim to restore joint function, minimise downtime and preserve athletic capability in horses worldwide.
