Esophageal Atresia and Tracheoesophageal Fistula Management

Summary

Esophageal atresia (EA) with or without tracheoesophageal fistula (TEF) represents a spectrum of congenital anomalies in which the continuity of the oesophagus is interrupted and may be abnormally connected to the airway. Initial management focuses on prompt diagnosis, airway protection and prevention of aspiration, followed by definitive surgical repair. Advances in neonatal intensive care, perioperative imaging and minimally invasive techniques have improved survival rates to over 90 percent in centres of expertise. Nevertheless, postoperative challenges—such as anastomotic tension, long-term dysmotility, gastro-oesophageal reflux and respiratory sequelae—demand a multidisciplinary approach. Recent efforts have centred on optimising tissue healing through cell-based therapies, refining mechanical characterisation to guide tension-free repair, and enhancing functional recovery via tailored feeding protocols. International networks and consensus guidelines now advocate for centralised care of complex variants, notably long-gap EA, to harness specialised surgical and supportive expertise. The global burden of EA/TEF underscores the need for accessible innovations that reduce morbidity, support normal growth and promote quality of life into adolescence and beyond.

Contemporary biomechanical investigations have employed ex vivo uniaxial stretching of lamb oesophageal specimens to elucidate ultrastructural alterations under varying strain levels. Observations reveal that elongation beyond 30 percent induces significant disruption of intra- and extracellular bonds, increased intercellular spacing and distortion of myofibrillar architecture. These findings furnish quantitative benchmarks for safe limits of tension during primary repair and inform the mechanical design of tissue-engineered oesophageal constructs. By mapping the relationship between applied force and microstructural integrity, this work lays the groundwork for translation of regenerative scaffolds capable of withstanding physiological stretch during growth and peristalsis.
