Esophageal Cancer Surgical Outcomes
Summary
Esophageal cancer remains a formidable challenge in global oncology, with surgical resection constituting the cornerstone of curative treatment for locally advanced disease. Outcomes after oesophagectomy are defined by a balance between oncological control and perioperative risk. Radical resection with adequate lymphadenectomy (often two- or three-field) offers the best chance of eradicating locoregional tumour deposits, but is associated with substantial morbidity, including pulmonary complications, anastomotic leakage and cardiovascular events. Advances in perioperative care, patient selection, neoadjuvant therapy and surgical technique have gradually reduced in-hospital mortality to below 5–10 per cent in high-volume centres. Long-term survival at five years now approaches 40–50 per cent among patients achieving R0 resection, although recurrence and late complications continue to impact quality of life. Minimally invasive and hybrid approaches have been developed to reduce operative trauma, and modern imaging techniques now aid intraoperative decision-making to improve conduit perfusion and minimise leak rates. Centralisation of care and multidisciplinary management have further enhanced outcomes by concentrating expertise and standardising protocols for enhanced recovery. As the field moves forward, the integration of robotic platforms, fluorescence-guided perfusion assessment and refined postoperative pathways promises further gains in safety and survival.

Minimally invasive oesophagectomy (MIO): Laparoscopic or thoracoscopic approach to oesophagectomy aiming to reduce surgical trauma and enhance recovery.
R0 resection: Complete tumour removal with microscopically negative margins, indicating no residual cancer at the cut edges.
Lymphadenectomy: Surgical removal of lymph nodes in the mediastinum and abdomen to assess and control regional metastatic spread.
