Summary
Hemodialysis represents a life-sustaining therapy for those with end-stage renal disease, yet it carries profound experiential challenges that extend well beyond the technical procedure. Patients commonly face cyclical physical and cognitive fluctuations, including intradialytic fatigue, hypotension and “dialysis fog,” which compromise daily functioning. Psychologically, the relentless regimen and dependency on a machine can precipitate anxiety, depressive moods and a pervasive sense of loss of autonomy. Socially, treatment schedules impose strict time constraints that disrupt employment, family roles and social participation, while stigmatisation around vascular access sites may undermine body image and intimacy. Self-care demands—spanning strict dietary regimens, fluid restrictions and vascular access maintenance—require significant adaptation and patient empowerment, yet are often shaped by variable support from caregivers and healthcare teams. Across diverse settings, qualitative research has identified common themes of resilience, adaptation and the critical role of tailored education, peer support and integrated psychosocial services in alleviating treatment burden. Global ageing trends and resource limitations further intensify these challenges, underscoring the need for holistic, patient-centred models that address physical symptoms, emotional well-being and social reintegration. Practical applications include flexible scheduling, telehealth monitoring, structured self-management programmes and family-centred counselling to strengthen coping strategies and enhance quality of life.
