Extrahepatic Metastasis in Hepatocellular Carcinoma

Summary

Extrahepatic metastasis in hepatocellular carcinoma (HCC) denotes the spread of primary liver malignancies beyond the hepatic parenchyma to distant organs. The most common sites include lung, bone and lymph nodes. The emergence of extrahepatic lesions signifies a more aggressive tumour biology, compounding the already complex management of HCC. Patients with metastases outside the liver often present with advanced symptoms and diminished liver function, limiting local treatments. Historically, systemic therapies have offered only modest benefits, and the prognosis remains poor, with median survival measured in months. Recent advances have begun to unravel underlying mechanisms of tumour dissemination, highlight predictive biomarkers and refine combined modality approaches. A clearer understanding of the metastatic cascade, from intravasation to colonisation of secondary sites, is informing the design of novel agents and locoregional strategies aimed at extending survival and preserving quality of life.

Palliative treatment: Therapy aimed at symptom relief and life-extension rather than cure, often employed in advanced-stage cancer.
