Summary

Falls are a leading cause of injury, hospitalisation and loss of independence among older people, affecting physical and psychological well-being across diverse populations. Risk emerges from an interplay of intrinsic factors—such as muscle weakness, impaired balance, gait disturbances, sensory deficits and chronic comorbidities—and extrinsic factors including environmental hazards, polypharmacy and social determinants. Traditional assessment relies on clinical performance tests (for example, the Timed Up and Go test and the sit-to-stand test), observational gait analysis and, increasingly, sensor-based measures of postural sway and gait variability. Advances in digital motion analysis and wearable technology now allow objective quantification of balance and mobility in real-world settings. Prevention strategies are multifactorial, combining tailored exercise programmes (notably strength and balance training), home hazard modification, medication review and vision correction, supported by community-based education. Evidence demonstrates that structured group or home-based interventions can reduce fall incidence and related injuries. Translating these findings into practice requires interdisciplinary collaboration, integration of novel assessment tools into care pathways and policy support to ensure equitable access to preventive services. Future research aims to refine predictive algorithms, address health disparities in fall outcomes and develop scalable models that link mechanistic insights with population health approaches.
