Fertility Awareness and Menstrual Cycle Monitoring
Summary
Fertility awareness encompasses a range of methods and technologies designed to monitor the menstrual cycle, identify ovulation and estimate the fertile window for purposes of family planning, contraceptive practice or general reproductive health assessment. Traditional approaches include charting basal body temperature, cervical mucus characteristics and cycle length. More recently, digital health tools such as smartphone applications and wearable sensors have enabled continuous monitoring of physiological markers—temperature, heart rate and respiratory rate—offering personalised insights into cycle regularity, phase transitions and potential anomalies. By integrating self-reported symptoms with objective measurements, modern fertility awareness seeks to empower individuals with non-invasive, data-driven guidance for conception or natural contraception. This field holds global significance by improving access to reproductive health information, reducing unintended pregnancies and contributing to population-scale understanding of menstrual health variations across demographic groups.

Fertility Awareness and Menstrual Cycle Monitoring publication trend
The graph below shows the total number of articles in fertility awareness and menstrual cycle monitoring across all publications each year (not limited to Nature Index journals).
Technical terms
Basal body temperature (BBT): Resting body temperature recorded immediately upon waking, rising after ovulation due to progesterone increase.
Follicular phase: The first half of the menstrual cycle, beginning with menstruation and ending at ovulation, characterised by follicle development in the ovaries.
Luteal phase: The second half of the menstrual cycle, extending from ovulation to the onset of the next menstruation, marked by progesterone secretion from the corpus luteum.
Fertile window: The period of highest fecundability, typically spanning five days before ovulation and the day of ovulation itself, when conception is most likely.
Heart rate variability (HRV): The variation in time intervals between heartbeats, which fluctuates across the menstrual cycle in response to hormonal changes.
