Summary
Fetal magnetic resonance imaging (MRI) has emerged as a vital adjunct to obstetric ultrasound in the evaluation of ventriculomegaly, the prenatal enlargement of the lateral cerebral ventricles. By offering superior soft-tissue contrast and multiplanar views without ionising radiation, MRI enhances detection of associated anomalies, refines aetiological classification and informs prognostic counselling. Fast imaging sequences mitigate motion artefact, while high-resolution T2 and T1 protocols delineate ventricular morphology, parenchymal development and haemorrhagic or migrational abnormalities. Quantitative approaches—ranging from two-dimensional measurements to three-dimensional volumetric segmentation—have begun to define normative trajectories and thresholds for intervention. Collectively, these advances support targeted genetic testing, guide perinatal management and facilitate multidisciplinary planning, with the ultimate aim of improving neurodevelopmental outcomes for affected fetuses.

One cohort investigation revealed that a significant proportion of children with antenatal isolated enlargement exhibit autism-spectrum traits, sustained attention deficits and executive-function impairments at school age, linking prenatal ventricular size to cortical overgrowth and later behavioural profiles. Another large-scale analysis of chromosomal microarray testing in fetuses with ventriculomegaly demonstrated a higher incidence of pathogenic copy-number variants in bilateral, non-isolated and severe presentations. This work underpins the recommendation of genome-wide array testing as a first-tier prenatal diagnostic tool, highlighting the stratification of risk based on laterality, isolation and severity parameters.
