Summary

Fetus-in-fetu is an exceedingly rare congenital anomaly in which a malformed parasitic twin resides within the body of its host. Typically detected in infancy or early childhood, the condition most often presents as an abdominal mass, although ectopic locations such as the sacrococcygeal region, intracranial compartment or scrotal sac have been documented. Embryologically, it is thought to arise from aberrant division of a totipotent blastocyst, resulting in inclusion of one twin within the other. Distinction from teratoma—a germ-cell tumour with malignant potential—is critical, hinging on the presence of organised axial structures such as vertebral segments and limb buds. Modern antenatal ultrasound, supplemented by computed tomography (CT) and magnetic resonance imaging (MRI), enables prenatal detection, delineation of vascular supply and assessment of adjacent organ compression. Surgical excision remains the mainstay of treatment, with open or minimally invasive approaches tailored to site and size of the mass. Long-term follow-up includes monitoring of tumour markers and imaging to detect rare recurrences or complications, ensuring excellent prognosis in most cases.
