Summary

Food insecurity—defined as limited or uncertain access to adequate, safe and nutritious food—remains a pervasive challenge with profound implications for physical and mental health worldwide. Insufficient dietary intake and lack of dietary diversity contribute to both undernutrition and nutrient deficiencies, heightening risks of stunting, wasting and anaemia in children, and exacerbating chronic conditions such as obesity, diabetes and cardiovascular disease in adults. Psychological stress generated by food scarcity has been linked to anxiety, depression and cognitive impairment, while trade-offs between food and other necessities can trigger toxic stress, especially in vulnerable families. Emerging evidence underscores how episodic or persistent food insecurity disrupts metabolic homeostasis through inflammatory pathways and dysregulated stress responses, compounding health inequities. Interventions spanning social protection, targeted subsidies and community support programmes demonstrate potential to buffer adverse outcomes, yet tailored, multisectoral strategies are needed to address the complex interplay of economic, environmental and social determinants that shape nutritional well-being across the life course.

Recent cohort analyses have quantified a graded association between severity of household food insecurity and the onset of metabolic disorders, showing that individuals experiencing chronic insecurity face higher incidence of type 2 diabetes and hypertension than those with transient episodes. Advances in population-scale biomarker profiling have revealed elevated inflammatory mediators—such as C-reactive protein and interleukin-6—in food-insecure adults, linking physiological stress to both cardiometabolic risk and depressive symptoms. A randomised pilot evaluated unconditional cash transfers to low-income households, demonstrating that modest income support led to measurable improvements in dietary diversity scores, reductions in ultra-processed food consumption, and modest declines in glycated haemoglobin over a six-month period. Collectively, these studies illustrate how precise measurement of dietary patterns and stress biomarkers can guide interventions that mitigate the health consequences of food insecurity.
