Summary

Gastrointestinal fungal infections encompass a spectrum of conditions arising when opportunistic or primary fungal pathogens invade the mucosal or submucosal layers of the digestive tract. Traditionally regarded as rare, these infections are increasingly recognised in both immunocompetent and immunocompromised patients, with presentations ranging from indolent eosinophilic masses to fulminant angioinvasive disease. Candida species remain the most frequently isolated yeasts, capable of translocating across an impaired mucosal barrier, whereas members of the order Mucorales and entomophthoralean fungi such as Basidiobolus spp. give rise to distinctive granulomatous or necrotic lesions. Clinical features often mimic inflammatory bowel disease or neoplasia, leading to diagnostic delays and inadvertent surgical interventions. Key determinants of outcome include timely histopathological identification—often signalled by characteristic Splendore-Hoeppli material—combined with culture or molecular assays to guide antifungal selection. Management typically involves a combination of surgical debridement and prolonged systemic therapy, most often with azole compounds or amphotericin derivatives. Emerging insights into the gut mycobiome have underscored the importance of fungal–bacterial interactions in maintaining mucosal homeostasis, and raised the prospect of microbiome-based diagnostics and therapeutics. As global travel and immunosuppressive therapies become more prevalent, the burden and complexity of gastrointestinal mycoses are set to increase, emphasising the need for heightened clinical vigilance and interdisciplinary research.

Recent studies have elucidated mechanisms by which Candida albicans exploits epithelial damage to penetrate the intestinal barrier, triggering systemic candidiasis. Work combining in vivo models and patient samples has highlighted the pivotal role of intestinal Th17 responses in constraining fungal overgrowth and preventing dissemination. Parallel metagenomic analyses have revealed that patients with inflammatory bowel disease harbour a distinct mycobiome signature, characterised by enrichment of potentially pathogenic yeasts and altered fungal diversity, which correlates with disease severity and responsiveness to biologic therapies. Advancements in single-cell transcriptomics have begun to map the interactions between mucosal immune cells and fungal elements, uncovering novel targets for host-directed interventions aimed at bolstering antifungal defence without broad suppression of microbial communities.
