Summary

Hypoparathyroidism, Deafness, and Renal Dysplasia (HDR) syndrome is an autosomal dominant disorder arising from mutations in the GATA3 gene, which encodes a zinc finger transcription factor crucial for the development and function of the parathyroid glands, inner ear and kidneys. Clinically, individuals present with hypocalcaemia due to parathyroid hormone deficiency, sensorineural hearing loss and a spectrum of renal anomalies ranging from dysplasia to functional impairment. The pathogenic variants span missense and nonsense mutations, frameshifts and splice site alterations, resulting in GATA3 haploinsufficiency or dominant-negative effects. Molecular studies have elucidated how disruptions in the dual zinc finger domains compromise DNA binding, protein–protein interactions and transcriptional regulation, yielding variable phenotypic expression even within families. Advances in genetic analysis, including next-generation sequencing and functional assays, have sharpened diagnostic accuracy and deepened insight into genotype–phenotype correlations, informing clinical management and genetic counselling worldwide.
