Recessive mutations in FAT1 were found to cause a combined glomerulotubular nephropathy characterised by podocyte foot process effacement and tubular lumen defects. Loss of FAT1 impairs RAC1/CDC42 activation, and administration of small GTPase activators partially rescues migration defects, identifying a targetable pathway.
Comprehensive screening of Japanese patients with severe proteinuria revealed monogenic variants in 30% of cases, predominantly affecting WT1, NPHS1, INF2, TRPC6 and LAMB2. Monogenic status predicted lower rates of complete remission under immunosuppression, underscoring the value of genetic diagnosis for treatment stratification.
