Gliomatosis Cerebri Diagnosis and Management Techniques
Summary
Gliomatosis cerebri is a rare, diffusely infiltrative glial neoplasm defined by extensive involvement of multiple cerebral lobes. Its presentation is often subtle, with nonspecific symptoms such as headache, seizures and cognitive changes, complicating early detection. Diagnosis relies predominantly on advanced neuroimaging, notably magnetic resonance imaging (MRI) with fluid-attenuated inversion recovery (FLAIR) sequences, which reveal widespread hyperintense signals without discrete mass effect. Complementary modalities—magnetic resonance spectroscopy (MRS) and positron emission tomography—provide metabolic and perfusion insights that guide differential diagnosis against encephalitis and other diffuse gliomas. Histopathological confirmation via stereotactic biopsy remains the gold standard, enabling molecular profiling that informs prognosis and therapeutic planning. Management is challenging: surgical resection is generally unfeasible, radiotherapy offers modest benefit and conventional chemotherapy has limited efficacy. Recent advances in alkylating agents, targeted therapies and immunomodulatory strategies are under investigation, emphasising a personalised, multidisciplinary approach. Despite poor overall survival, incremental improvements in imaging accuracy and molecular classification are refining patient stratification and may pave the way for more effective interventions.
