Glucagon-Like Peptide-1 Receptor Agonists in Neurodegenerative Disorders
Summary
Glucagon-like peptide-1 receptor (GLP-1R) agonists, originally developed for glycaemic control in type 2 diabetes, have emerged as promising agents for the treatment of neurodegenerative disorders. These peptides cross the blood–brain barrier and engage receptors widely expressed in central nervous system regions implicated in motor and cognitive function. Preclinical studies demonstrate that GLP-1R activation modulates neuroinflammation, enhances mitochondrial function and synaptic plasticity, and reduces accumulation of pathological proteins such as α-synuclein and amyloid-β. Parallel epidemiological work suggests that diabetic patients receiving GLP-1R agonists may exhibit lower incidence or slower progression of Parkinson’s and Alzheimer’s diseases. Current research is extending these findings into controlled clinical trials of long-acting GLP-1 analogues, with the aim of repurposing a well-tolerated drug class to slow or halt neurodegenerative processes. The global burden of neurodegeneration, ageing demographics and the urgent need for disease-modifying therapies underline the significance of this approach.

Nigrostriatal pathway: The dopamine-transporting neural projection from the substantia nigra to the striatum, critical for regulation of movement and commonly degenerated in Parkinson’s disease.
Neuroinflammation: The activation of the brain’s innate immune response, involving microglia and astrocytes, which can contribute to neuronal injury when chronically sustained.
Cohort study: An observational epidemiological design in which a defined group of individuals is followed over time to assess the relationship between exposures (e.g. diabetes) and outcomes (e.g. neurodegenerative disease).
