Summary

Granulomatous inflammatory conditions of the breast constitute a group of benign yet often challenging disorders characterised by the formation of non-caseating granulomas within breast parenchyma. These lesions may arise in the absence of identifiable infection, as in idiopathic granulomatous mastitis (IGM), or in association with recognised causes such as foreign bodies, systemic granulomatous diseases or specific infections. Clinically, patients present with firm masses, skin changes, tenderness and, occasionally, sinus formation or abscesses, frequently mimicking carcinoma. Diagnostic confirmation relies on core needle biopsy with histopathological demonstration of epithelioid histiocytes, multinucleated giant cells and lymphocytic infiltrates. Management remains controversial and must be tailored to underlying aetiology and disease stage, ranging from conservative medical therapy—including corticosteroids or immunomodulators—to surgical intervention for refractory or advanced cases. The global impact of these conditions spans diagnostic uncertainty, variable practice patterns and the need for multidisciplinary coordination to optimise outcomes.
