Summary

Halitosis, commonly known as bad breath, affects a significant proportion of the global population and has both intra-oral and extra-oral origins. Diagnosis rests on sensory assessment and instrumental measurement of odorous compounds. The organoleptic test remains the clinical gold standard, complemented by gas chromatography and portable sulphide monitors for quantifying volatile sulphur compounds (VSCs). Recent advances in molecular profiling have highlighted the role of specific oral microbial communities, particularly on the tongue dorsum, in generating malodorous metabolites. Management encompasses mechanical debridement of the tongue and interdental spaces, chemical approaches such as antiseptic or oxidising mouthwashes, and systemic interventions targeting underlying diseases. Emerging strategies include modulation of the oral microbiota through transplantation or probiotic supplementation and targeted antimicrobial therapies such as photodynamic treatment. Integration of diagnostic precision and personalised management is critical to improving patient outcomes and mitigating the social and psychological burden of halitosis.

Recent studies have elucidated the link between periodontal health and halitosis by measuring hydrogen sulphide and methyl mercaptan levels in patients with gingivitis and periodontitis. Quantitative gas-chromatographic analysis has established sex-specific VSC thresholds for diagnosis and demonstrated that periodontal disease increases halitosis prevalence by more than threefold. Complementary work in paediatric cohorts has mapped the tongue-coating microbiome, revealing greater bacterial richness in affected children and shifts in co-occurrence networks that favour enhanced VSC production and reduced microbial consumption of odorous gases. These findings underscore the importance of both microbial ecology and host factors in the pathogenesis of intra-oral bad breath.
