Summary

Heavy metals such as lead, cadmium, mercury, arsenic and chromium are naturally occurring elements that have become pervasive pollutants owing to industrial, agricultural and urban activities. They enter human populations via contaminated air, water, soil and food chains, and may also arise from occupational and lifestyle sources. Owing to their persistence and non-degradable nature, these elements bioaccumulate in tissues such as bone, liver and kidney, causing multi-organ toxicity. At the cellular level, heavy metals induce oxidative stress through reactive oxygen species, disrupt enzyme function, alter DNA repair and trigger epigenetic modifications. In vulnerable groups—including children, pregnant women and workers—chronic exposure is linked to neurodevelopmental deficits, cardiovascular disease, renal impairment, bone disorders and various cancers. Globally, low- and middle-income countries bear a disproportionate burden, both in terms of health impact and economic cost. Surveillance programmes, biomonitoring strategies and regulatory measures have reduced exposures in many regions, but emerging technologies—such as phytoremediation, advanced chelation agents and nanomaterials for contaminant removal—offer new avenues for risk mitigation. An integrated approach, spanning environmental management to clinical intervention, remains essential to address this enduring public health challenge.

Epigenetics: Heritable changes in gene expression that do not involve alterations to the DNA sequence.

Chelation therapy: The administration of chelating agents to bind and facilitate the excretion of heavy metals.

Phytoremediation: The use of plants to absorb, sequester or detoxify environmental contaminants, including heavy metals.
