Summary
Helicobacter pylori is a spiral-shaped, gram-negative bacterium that colonises the human stomach and is recognised as a major risk factor for the development of gastric adenocarcinoma. Paradoxically, a growing body of evidence indicates that the presence of H. pylori at diagnosis may confer a survival advantage in patients with established gastric cancer. This protective effect is thought to arise from chronic immune activation and modulation of the tumour microenvironment, which can enhance host antitumour responses and influence sensitivity to chemotherapy. Prognostic assessments now increasingly consider H. pylori status alongside standard clinicopathological factors such as tumour stage, histological subtype and lymph node involvement. Geographical variations in bacterial strains and host genetics further complicate the picture, highlighting the need for region-specific strategies. Practical applications include tailored surveillance of high-risk populations, stratification of patients for adjuvant or neoadjuvant therapies and exploration of immunological biomarkers for personalised treatment planning.
