Summary

Hematidrosis and hemolacria are exceptionally rare bleeding disorders characterised by the passage of blood through intact skin or ocular secretions, respectively. These phenomena most often arise in the setting of extreme emotional or physical stress, though idiopathic presentations are also reported. The prevailing hypothesis proposes that sympathetic hyperstimulation induces intense vasoconstriction of perisudoriferous and periorbital capillaries, followed by reflexive vasodilation, capillary rupture and extravasation of blood into sweat gland ducts or lacrimal apparatus. Clinically, affected individuals present with episodic blood-tinged sweat, tearfulness with haemorrhagic discharge and, less frequently, bleeding from the ears or nasal mucosa. Diagnosis hinges on exclusion of trauma, coagulopathy, infection and neoplastic processes, guided by detailed history, laboratory evaluation and imaging. Management remains largely supportive; recent reports suggest that β-adrenoceptor antagonists and stress-reduction strategies may attenuate episode frequency. The scarcity of cases underscores the need for collaborative registries and mechanistic research to inform standardised care pathways.

β-Adrenoceptors: G-protein-coupled receptors on vascular endothelium and smooth muscle that mediate the effects of catecholamines, targeted by β-blockers to modulate vasomotor tone.
