Hepatic Portal Venous Gas Detection and Management

Summary

Hepatic portal venous gas (HPVG) is characterised by the presence of gas within the portal venous system, most commonly identified on cross-sectional imaging such as contrast-enhanced computed tomography (CT). Although historically associated with high mortality in the context of mesenteric ischaemia and bowel necrosis, improved imaging sensitivity and clinical stratification have revealed a broader spectrum of underlying causes, ranging from benign post-procedural gas entry to life-threatening sepsis. Early detection relies on high-resolution CT or Doppler ultrasound, often in conjunction with clinical signs such as abdominal pain, peritoneal irritation, lactic acidosis and raised inflammatory markers. Management now follows a risk-adapted algorithm: patients without evidence of bowel compromise may be treated conservatively with bowel rest, supportive care and antibiotics, whereas those showing signs of intestinal pneumatosis, poor mural enhancement or systemic instability require urgent surgical evaluation. Emerging approaches include quantitative CT perfusion analysis and machine-learning models to predict ischaemia, facilitating timely intervention and reducing unnecessary laparotomies. Globally, enhanced awareness of HPVG’s varied aetiologies has reshaped practice guidelines and emphasises multidisciplinary collaboration for optimised patient outcomes.
