Summary
Hepatocellular carcinoma arising in a non-cirrhotic liver represents a distinct clinical entity characterised by tumour development in the absence of advanced fibrosis. Although historically uncommon, its incidence is increasing, driven in part by metabolic liver diseases, viral hepatitis in patients without cirrhosis, and cryptogenic mechanisms. These tumours often present at a later stage owing to lack of surveillance in non-cirrhotic populations, resulting in larger lesion size and higher symptom burden at diagnosis. Surgical resection remains the cornerstone of curative therapy, with outcomes generally favourable when detected early and resected radically. However, heterogeneity in aetiology, tumour biology and immune milieu poses challenges for screening strategies and adjuvant treatment. Current research focuses on refining risk stratification, developing novel biomarkers and tailoring multidisciplinary approaches to improve early detection and patient survival globally.

These tumours display advanced stage at presentation, heightened systemic inflammation marked by elevated interleukin-6, and molecular profiles that parallel those seen in fatty-liver-associated HCC. Comparative immunoprofiling indicates overlapping biomarker signatures and genetic risk variants, such as PNPLA3, suggesting common pathways in cryptogenic and non-alcoholic fatty liver disease-related HCC. These insights underscore the need for targeted surveillance and biomarker development.
