Hepatocellular Carcinoma Surveillance and Detection Strategies
Summary
Hepatocellular carcinoma (HCC) ranks among the leading causes of cancer-related mortality worldwide, reflecting both the global prevalence of chronic liver disease and the challenges of detecting tumours at a curable stage. Surveillance is targeted at individuals with established cirrhosis or other high-risk conditions, typically at six-month intervals, to identify early lesions amenable to curative therapy. Conventional approaches combine biannual abdominal ultrasound with serum alpha-fetoprotein measurement, balancing affordability and accessibility against variable sensitivity for lesions below 2 cm. Cross-sectional imaging by contrast-enhanced CT or MRI offers higher accuracy but at increased cost, radiation exposure or limited availability. Recent efforts seek to refine patient stratification by integrating novel circulating biomarkers, risk-prediction models and quantitative image analysis. Abbreviated MRI protocols and emerging radiomic or deep-learning algorithms applied to routine scans promise to enhance lesion conspicuity while reducing examination time and operator dependence. Complementary strategies under investigation include liquid-biopsy assays for circulating tumour DNA, risk-adaptive surveillance intervals and point-of-care elastography to personalise monitoring. Together, these advances aim to shift the diagnostic paradigm from population-based screening to precision surveillance, thereby improving early detection rates, broadening access in resource-limited settings and ultimately reducing HCC mortality on a global scale.
