HIV Testing Strategies and Community Interventions
Summary
HIV testing remains the cornerstone of global efforts to diagnose infection early, link individuals to care and reduce onward transmission. Strategies range from facility-based opt-out screening to community-led approaches that harness local networks, digital media and peer support. A growing shift towards decentralised and client-centred models has emphasised self-testing, mobile outreach and social marketing to reach marginalised populations. Strength-based interventions build on internal and external developmental assets to foster personal responsibility and positive identity, thereby promoting sustained engagement with testing and prevention services. Community interventions also address structural barriers—stigma, confidentiality concerns and access challenges—through partnerships with trusted leaders, tailored messaging and integration of testing into everyday venues. The combined application of biomedical tools, behavioural insights and community mobilisation underpins a comprehensive public health response that is adaptable across diverse global settings.
Research from Nature Portfolio
Recent studies have shown that strengthening developmental assets among young Black men who have sex with men can significantly increase both HIV testing and uptake of pre-exposure prophylaxis. External supports such as family and other adult encouragement were associated with greater personal responsibility, which in turn drove higher rates of testing and PrEP use. Path analyses underscore the importance of positive identity and internal self-efficacy as mediators of prevention behaviours. These findings have informed calls for strength-based community interventions that cultivate social support networks and empower at-risk youth to engage proactively with HIV services.

Technical terms
Developmental assets: Internal (e.g. personal responsibility, positive identity) and external (e.g. family support, mentorship) resources that bolster an individual’s capacity to engage in health-protective behaviours.
Pre-exposure prophylaxis (PrEP): A preventive antiretroviral medication taken by HIV-negative individuals at high risk to reduce the likelihood of acquiring HIV.
HIV self-testing: A process whereby individuals collect their own specimen (oral fluid or finger-prick blood), perform an HIV test and interpret the result privately.
Opt-out testing: Routine HIV screening offered in healthcare settings whereby patients are tested unless they explicitly decline, removing the need for separate consent procedures.
Social marketing: The application of commercial marketing principles—audience segmentation, tailored messaging, consumer research—to influence behaviours for public health benefit.
