Home-based HIV testing and counselling strategies have emerged as a pivotal approach to increase diagnosis rates, particularly in communities with limited access to health facilities. By delivering rapid tests and personalised counselling directly to households, these interventions aim to reach individuals who may not engage with clinic-based services, including men, young adults and marginalised groups. Home-based programmes often combine door-to-door visits with self-testing options, point-of-care diagnostics and linkage support to ensure prompt initiation of prevention or treatment. This model has demonstrated high uptake, early detection at higher CD4 counts and reduction of barriers related to travel, stigma and time constraints. The global significance lies in its potential to contribute to ambitious targets for diagnosis, treatment initiation and viral suppression, while tailoring services to local contexts and promoting equitable access.

A foundational systematic review and meta-analysis evaluated community versus facility-based HIV testing modalities across sub-Saharan Africa, comparing home, mobile and campaign strategies with traditional clinic approaches. Community testing achieved markedly higher coverage and earlier diagnosis, identifying a greater proportion of individuals at higher CD4 counts and with fewer symptoms. Mobile units reached a larger share of men, while door-to-door self-testing reached more young adults. When paired with facilitated linkage—such as follow-up by counsellors—home-based approaches yielded linkage rates approaching 95 per cent and same-day antiretroviral initiation in three-quarters of cases. The analysis underscored the value of integrating self-testing, outreach to key populations and active linkage mechanisms to maximise both prevention and treatment impact.
